The mission of Prosperity House of Lynchburg, Inc., is to provide a quality of care for each individual by supporting them with physical, mental, and emotional needs in a family home environment where the individual can live a normal life as possible.
Services provided by Prosperity House of Lynchburg, Inc., will accept men and women who are at least 18 years of age with a primary diagnosis of intellectual disabilities which may be accompanied with mental illness. Individuals may be non ambulatory and/or ambulatory. Individuals with a continuous need for nursing care will not be served in Prosperity House of Lynchburg, Inc. If the individual seeking residence meet the above criteria then a meeting will be held with the Support Coordinator, authorized representative and/or family member with the individual to determine eligibility for services. After concluding that Prosperity House of Lynchburg, Inc., can provide services for the individual, opportunity for visitation, and to spend time in the home will be established

Prosperity House Of Lynchburg (PHL) will collect, maintain, and report and make available to DBHDS (“The Department”) the following information:
· Each allegation of abuse or neglect to the Department.
· Instances of seclusion or restraint shall be reported to the Department.
PHL will submit, or make available, reports, and information that the Department requires to establish compliance with these regulations and applicable statutes. Records that are confidential under federal or state law shall be maintained as confidential by the department and shall not
be further disclosed except as required or permitted by law. However, there shall be no right of access to communications that are privileged pursuant to § 8.01-581.17 of the Code of Virginia. Additional information requested by
the Department if compliance with a regulation cannot be determined shall be submitted within 10 business days of the issuance of the licensing report requesting additional information. Extensions may be granted by the Department when requested prior to the due date, but extensions shall not exceed an additional 10 business days. PHL, may not submit any misleading or false information to the Department.

We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and support. We will take the time to get to know you and develop an individualized service plan that fits your specific needs.
Transportation is provided for all community outings and medical appointment. PHOL, Inc., has 2 wheelchair accessible vehicles.
